New Patient

To register with the practice please use this link https://gp-registration.nhs.uk/L83100. You will need to do one form for each person who is registering.

Once you have completed the forms this will electronically be sent to the surgery, and we will register you with the practice within 7 working days of receiving the request. If there are any issues with the registration one of the team will contact, you within this timescale.

THE PRACTICE MAY NOT ACCEPT YOUR REQUEST FOR OUT OF AREA REGISTRATION THIS WILL BE DEPENDANT ON YOUR HEALTH CARE NEEDS AND DISTANCE FROM THE PRACTICE.

If you live in PL1, PL2, PL4, PL5, PL10, PL11, PL12, PL13, PL14, PL15, PL16, PL17, PL18, PL19 or PL20 postcodes, you will be told to register elsewhere. You must use the GP map to check if you are in the Beacon Medical Group area link as per below.
