Urgent Care Team
We have a multi-skilled urgent care team to support patients with urgent care issues. Our team is made up of ACP (Advanced Clinical Practitioners). ACPs are higher experienced and skilled professionals, they have the skills required to assess, diagnose and treat patients healthcare needs and can refer to an appropriate specialist if required. ACP’s come from an array of professional backgrounds including Nursing, Paramedic, Physiotherapy and Paediatric nursing. All are experienced and highly qualified and have worked in a multitude of specialist departments within the community and hospital environment.
The team offers same-day urgent care appointments via the telephone, online and face to face where required.They are able to treat and prescribe medication for patients with an array of acute medical conditions of all ages.
Our Urgent care Team Lead is Simon Robinson.
Nurse Team
Our practice nurses have particular skills in health promotion, contraception, travel advice, immunisations (such as children’s, seasonal and age-related vaccinations), annual reviews for chronic diseases such as diabetes, asthma, heart disease, cervical smears and caring for wounds.
Nursing Associates, Health Care Assistants and Phlebotomists work alongside our practice nurses and carry out blood pressure checks, blood tests, ear care and they provide removal of stitches and staples (supporting secondary care with follow up appointments). If the nurses feel that you need to see a doctor, they will refer you on. Equally, you will often find that the doctors refer you on to the nurses.

Pharmacy Team
Our pharmacy team is a team of Clinical Pharmacists and Pharmacy Technicians working to improve medication safety and management, they have detailed expert knowledge in medicine issues and consult with patients in person and by phone. The team deal with medication reviews, medicine reconciliation, ensuring correct monitoring of high-risk drugs and discussing medication compliance with patients. The team also support with the management of long-term conditions.

Administrative Team
The teams are supported by valued members of our administration teams. Our administration staff are pivotal to the safe management of correspondence and letters and supporting patients to access the right care. Our administration teams support our GPs with coding letters, ensuring patients have their care reviews such as long-term condition reviews and cervical screening, as well as typing and sending onward referral letters and booking and signposting patients to the right care.
The team are our key link between external services such as secondary care, private providers and any other health provider offering NHS services.
The Prescription Hub Team deal with medication requests from all six of our surgeries. The team are skilled in looking out for any reviews that are needed for regular medications and getting these reviews booked. The Prescription Hub work closely with our Pharmacy Team to ensure our patients’ medications are being dealt with and issued efficiently and appropriately.
Musculoskeletal Team
Our Musculoskeletal team (MSK)consists of two parts; The First Contact Physiotherapy (FCP) service and the GP with special interest (GPwSI) led intermediate care service. The FCP service allows rapid access to experienced Physiotherapists who form part of our Urgent Care Team.
The GPwSI intermediate care service was set up 6 years ago to act as a ‘halfway’ clinic between Primary Care and Orthopaedics. The majority of MSK patients do not need operations. Rather they need to see an experienced clinician with access to specialised imaging so a correct diagnosis can be made and evidenced based advice given on the best treatments available.
Social Prescribing Team
Social Prescribing Link workers offer people time to explore those factors which are affecting their health and to find out what really matters to them; they take a holistic approach, supporting people to take greater control over their own health and wellbeing. Social Prescribing Link workers, link into the local communities in the areas where they work. They have knowledge and connections with many local groups and organisations. The Social Prescribing Link Workers role is to provide advice and guidance, and through getting to know the person to connect them to those community groups and statutory services the person feels would be helpful to them, for any practical and emotional support needed.
Care Home Team
At Beacon we have a dedicated multidisciplinary team (MDT)with the lead GP, Clinical Pharmacist, Visiting Clinical Practitioner, and a Care home co-ordinator to support the health of care home residents, whether physical or mental. Our dedicated care home team deliver routine ward rounds to assess individuals and review their personalised care and deal with any acute illness. The MDT works with the GP to ensure that residents with a learning disability who are recorded on their GP’s register obtain their free annual health check, which can improve their health by identifying health problems earlier and managing existing health conditions.
